Request a sick note (Fit Note) for work

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or via the Gov.UK website
If you are off sick for more than seven days, and think that you need a Fit Note, you will need to have an appointment with a doctor. If a follow-on Fit Note is required, you may be able to request this online, using PATCHS. If you are unable to use the PATCHS online consultation tool to request a repeat Fit Note then please contact the surgery via phone. You may have to speak to a doctor regarding an extension to a Fit Note.
